Instructions for getting your NYSWYSA (US Youth Soccer) Player Passes
- Email me (registrarEUSA@rochester.rr.com) a list of players to be rostered. The list/database should include the player’s full name, address w/zip code, phone number, date of birth, and jersey number (if possible). Include coaches and managers names and complete addresses, as well as email addresses.
Please note: I now need to verify DOB of any new player before entering the player into the system. I will check the NYSW RosterPro system to determine which players I will need to see proof of birth for. Once I receive proof of birth for those players, I will then enter your team into the NYSWYSA website, and will print passes and a roster. Indicate in your email that you want NYSW passes.
- Rosters are certified when printed. I no longer need to stamp them. I will give you one copy. Please make as many copies as you will need for the season.
- Have players sign player passes exactly as their name appears on the front of the pass. Glue a 1”x1” photo of each player to their pass. Drop off above to the box on my front porch.
- I will certify and laminate player passes, and notify you when they are ready for pick-up. EXCEPTION – Teams participating in the RDYSL League will need to have passes certified by the RDYSL Registrar’s (See RDYSL website for this information).
- Please keep in mind that ANY changes to your original roster need to have a Player/Team Status form completed for EACH change. This includes transfers to another team, as well as release from team roster. When a player is released from a team, the player pass must be surrendered with the Player/Team Status Form. Refer to www.nyswysa.org under “Forms” and then select “Player/Team Status Form”.
- A Release to Secondary Team Form must be completed when a player is rostered on more than one team. This form requires several signatures, and there is a fee associated with this process, which will be assessed in RosterPro. Refer to www.nyswysa.org under “Forms” and then select “Release to Secondary Team Form”.
- All coaches, assistant coaches, managers and volunteers associated with the team must complete an on-line Disclosure Statement for Risk Management clearance. Once I have entered your data into the RosterPro system (to do this I need your name, address, phone number and preferred email address), I will have an automated email sent to you with a link to the application. Please complete the application and submit it. Your pass will be mailed to me, and I will alert you when it arrives.

house as you are facing it, next to the sliding glass door.
NOTE: It can take up to two weeks from the time Gail receives the information, until the pass process is complete. PLEASE PLAN ACCORDINGLY!
Always indicate your team name (ie., BU12 Blue) and your role with the team (ie., manager), in all emails.
2011-12 Registration costs:
- $13.50 for Player Registration and Insurance
- $6 for Staff Risk Managerment Pass
- $25 for Replacement Pass for staff members.
